How do I get referred to this service?
Children: The service accepts referrals for children from their parents and guardians.

You need to print out this form, fill it in and send it back to the address on the form.
The service also accepts referrals from GPs (doctors) and other health professionals.
Adults: Referrals for adults must be made by a medical doctor or healthcare professional.
